Collection and Use of Health Information.

I understand that Ollio Lactation collects health information to:

• Assess, plan, and deliver lactation and infant feeding support

• Maintain accurate health records

• Support administrative functions such as scheduling, billing, and record retention

The collection, use, and disclosure of my health information is authorized under sections 18–24, 25–30, and 33 of the Health Information Act (HIA).

Client Access.

All health information collected by Ollio Lactation is stored securely in the Practice Better Client Portal. Clients have full access to their information and may choose to share it with other healthcare providers if they wish.

Home Visits and Safety.

I understand that Lauren Paré will be coming to my home, and I grant permission for her to share my address with her spouse to ensure her safety. I also understand that Lauren Paré will use GPS on her personal device to navigate to my home.

Payment Policies.

I have read and reviewed Lauren Paré’s payment policies and understand that I am responsible for all charges associated with this visit at the time services are rendered. Lauren Paré may communicate with my credit card company, bank, or insurance provider for any payment-related matters. It is my responsibility to provide accurate and current payment information.

Secure Messaging and Communication.

Lauren Paré provides a secure messaging portal through Practice Better. I understand that I am not obligated to use this secure messaging platform. If I opt out and initiate communication by phone, text, or email, Lauren Paré will respond accordingly. I understand that phone, text, and email are not secure means of communication, and I give permission for Lauren Paré to send and receive calls, texts, and emails that may contain my Personal Health Information (PHI) if communication is initiated by me.

Access and Questions.
